Activity Registration Daily Screening Form Quick Start Guide
From the Public View, Parents will be able to fill out the Daily Health Screen Form by clicking on the Covid-19 Daily Screening Forms tab and selecting an activity from the list.
Proceed to fill out the form and then click Submit.
To see a report of the submitted forms, log in as Administrator, go to the Pages tab and click on the activity name.
Then, go to the Results tab and click on Table for more detailed information.
To download the report as an Excel file, go to the Download tab, select the components to include in the Report, and click Download.
